Urinary incontinence is the involuntary loss of urine, which is a social or hygienic problem. Urinary incontinence is a distressing condition which although rarely life threatening, severely and adversely affects all aspects of a woman’s quality of life.
The pattern of inconvenience of urine in women differs greatly between the developed and less developed countries. In the developed countries, urinary incontinence is commonly associated with pelvic relaxation. In less developed countries including West Africa sub- region, it is commonly due to childbirth related injuries that result in urinary fistulae formation. This affects about 100,000 women yearly and the affected form part of difficult incontinence group that needs specialized care. Incontinence due to pelvic relaxation found in this area are considered as minor problem when compared with incontinence secondary to obstetrics trauma.
In women with healthy lower urinary tract, urine only leave the bladder via the urethra when the intravesical pressure exceeds the maximum urethral pressure. In case of urinary incontinence not due to a fistula, the bladder pressure exceeds the urethral pressure because the urethral sphincter mechanism is defective as occurs in genuine stress incontinence or because the detrusor pressure is excessively high as occurs in detrusor instability.
Women suffer incontinence more twice often as man. Late complications of pregnancy and child birth, post menopausal estrogen deficiency and the very structure of the female urinary tract account for this difference. Older women experience incontinence of urine more often.
Urinary incontinence are classified into forms like ;
Genuine Stress incontinence is a when small amount of urine comes out when coughing or sneezing or during exercise
Urge incontinence is leakage of large amount of urine at unexpected times following detrusor muscle activity.
Overflow incontinence is the leakage that occurs due to full bladder.
Functional incontinence occurs in immobility.
Mixed incontinence is the combination of both stress and urge incontinence.
Transient incontinence is temporary leakage of urine due to infection or medication.
Fistulae :This is total incontinence of urine due to abnormal communication from the urinary bladder.
